Book an Appointment

We provide 3 convienient ways of making an appointment request to our office. Of course you can always call (970) 948-9128 and we will gladly help you over the phone during regular business hours.

You can also book an appointment via email. Click Here and you will be prompted to create an email to our receptionist. This email should include your name and phone number, the dates you are available and the time frame. And then optionally can include specific requests for your appointment such as the type of visit or the reason for your visit.

The third option is our appointment request feature which allows you to send us the neccessary details outlining your appointment request by simply filling out the following form.
